Please include all necessary information for the referral, as follows:

• Patient’s full name and date of birth.
• Contact information - including the patient’s phone number and email address.
• Insurance policy and numbers
• Social Security number
• Reason for consultation
• All relevant radiographic images. 
  • It is extremely helpful if the patient can bring a disc with their images on it to the office for their appointment.

We are available for same day appointments if you feel that urgency is required.

We offer several same-day procedures including drainage of abscesses, laceration repair, ultrasound guided breast and lymph node biopsies, ultrasound guided thyroid biopsies, skin biopsies, and cyst and skin cancer excisions.
